Thank you for choosing our office for your child's dental care. Please know that we are committed to providing the best care for your child, that we have policies in place that allow us to provide the level of care that your child deserves.
To maintain the practice operations and prevent potential misunderstandings, we ask patients to accept and adhere to the following office policies regarding their dental treatment. Please feel free to contact us with any questions you may have.
Appointments
To maintain the level of care that we provide, we do not double-book appointments. This helps to keep our office running in a timely fashion, and also gives you and your child the most time to spend with our clinical staff. For this reason, we ask that you give us 48-hour notice to reschedule or cancel an appointment. For missed appointments or appointments rescheduled or canceled with less than 48-hour notice there is a $50 fee. After two missed appointments we may ask that you seek care from another dental provider. Our appointment policy is not a threat or a punishment, it simply allows for us to run an effective and efficient office and makes appointment times available for those who need them. If you are 15 minutes late for your child's appointment, we may reschedule the appointment in order to keep the following families running on-time.
Outstanding Balances
If you are billed for any outstanding balance, please be aware that balances carried over 30 days will be charged a rebilling fee of $10 per monthly billing statement. While we choose not to send families to collections for unpaid bills, we do require prior balances to be paid in full before scheduling another appointment.
Insurance and Payment
We accept most PPO dental insurance. We are an out-of-network provider for all PPO plans except Delta Dental Premier. This means that your insurance will pay what it has arbitrarily determined to be the “usual and customary rate” for dental treatment. We try to run as efficiently as possible, which helps to keep your cost low, and many times we fall within your insurance companies UCR. We do our best to estimate your insurance benefits. You are responsible for your deductible and ESTIMATED co-payment on the day of treatment. You are responsible for any balance not paid by your insurance company after your claim has been paid.
As a courtesy to you, we will submit your insurance claim for you. This includes any required documentation, copies of x-rays, and letters of necessity. This does not guarantee any benefit, and we do not make benefit determination decisions. Your insurer makes all benefit determinations. It is your responsibility to understand what benefits are offered by your dental plan. We are available to answer any questions you may have about your dental insurance.
Financial Information
We realize that every person’s financial situation is different. For this reason, we have worked hard to provide a variety of payment options to help your child receive the dental care that he needs and deserves that allows him to enjoy a healthy, beautiful smile. Dental treatment is an excellent investment in an individual’s medical and psychological care. Children with healthy teeth can sleep better, learn better, and feel better. We are always available to answer your questions or assist you in any way we can.
Should your child require any dental services beyond routine exams and cleanings, you will be given an explanation of treatment along with a written treatment plan. This includes an itemized breakdown of fees for each procedure, as well as an estimate of your insurance benefit. Any difference in price between our fee and your estimated insurance payment is due at the time of service. We can submit your child's treatment plan to your insurance company for pre-estimation. This will allow us to better estimate the insurance payment.
As a courtesy to you, we will submit all required paperwork to your insurance company for reimbursement. We will also estimate your balance due after insurance. This balance is due on the day of service. Any remaining balance after insurance payment will be billed to you; any credit will be refunded promptly, or applied to your account at your request. We accept cash, checks (under $500.00), debit cards, and all major credit cards.
Divorce Decrees
This office is NOT party to your divorce decree. The responsibility for minors rests with the accompanying adult.

Permanent Teeth and Baby Teeth 
You and your child are so excited about losing that first tooth, but then the permanent tooth starts coming in and the baby tooth hasn’t fallen out yet, and he starts to look like Jaws! Don’t worry, this is very common, and it almost always resolves naturally.
We Will Monitor Your Child's Baby Teeth
During an appointment at Palos Pediatric Dentistry, I would want to check to be sure that they baby tooth is at least a little mobile. Then I would monitor at their check-up visits to be sure that the baby tooth is loosening more and more. Most often, the baby tooth falls out on its own. After it does, pressure from the tongue helps to move the permanent tooth forward, toward the correct position.
